The individuals and families that we serve are at high risk for
a number of poor health, outcomes due to several
compounding factors, including: poverty, social isolation, lack
of transportation, lack of health insurance, low levels of
education, cultural and linguistic barriers, poor diet, a lack of
knowledge of risk of substance abuse, breast cancer,
HIV/AIDS, and birth defects, and a lack of familiarity with and
trust of the U.S. health care system.
Our programs are based on a community-centered model of
increasing the awareness and knowledge of community
members of key health and wellness issues so that they can
access the resources they need for themselves and their
families.
